    Hi,

    I am not _too_ sure what happened with your system, but

    a) 'update-menus'

       should rebuild your menu

    b) The system menu layout is given in /usr/lib/menu, where you can have
       a look for the structure. Then you can take entries from there,
       adjust them to your liking and put them into ~/.menu. After the next
       'update-menus', you will have your customized entries available.
