Try getting a newer version of K3B, I'm running 0.12.15 here. K3B has
had a lot of bug fixes. And don't force DAO writing mode, I just set
it to AUTO.

It happens often here too, as k3b does not frequently erase/write over
the contents already in the disk. Maybe, you should try to use
growisofs directly. I have never experienced any problem with
growisofs. Look at:
