[kde] modifying kde's right click menu



Hi
When I right click an item in Konq or search the behaviour of "open with"
changes sometimes it offers what I did last, for example open with kate, on
certain things it doesn't. How can I change things so that the previous open
withs are allways shown?
Also how can I add additional commands to the right click operations menu?
For example open with kate. That would be my preferred solution.

PS The spell check seems to work every where it should apart from the
subject line of an email?
