I'm feeling stupid, it suddenly works with the following settings:

$ cat ~/.kde/share/apps/kdesktop/programs/xstarfish.desktop
[KDE Desktop Program]
Command=xstarfish -g %xx%y -o %f
Comment=Beautiful Math
Executable=xstarfish
PreviewCommand=xstarfish -g %xx%y -o %f
Refresh=5
