    What application are you using to create the text? Word processors can
    create forms, where you fill in the data. Also, one trick I used a
    while ago, is simply create a form with the appropriate boxes and print
    it. Then run it through the printer again to add the data.

    Or you could import the data into a spread ***. All sorts of ways,
    depending on your situation.
