    It's repeatable under Linux but not under Windows, where it will fail
    unpredictably, right?

    So the failure mode is completely repeatable under Linux but it is
    unpredictable under Windows?

    Perhaps you do have some bad hardware, though it's hard to what what
    (other than the fact that MemTest86 is giving the RAM a pass).

    You could get the Ultimate Boot CD (<http://www.ultimatebootcd.com/>)
    and see if it contains a suitable disk diagnostic (it includes many
    vendor-specific hardware diagnostic programs).
