I assume the Current State: Unavailable for all usb devices is my
problem but I'm not sure how to correct it. I don't know if it is
relevant or not but the first entry (Kensington Expert Mouse) works and
it always has, even with the OSE version.

Make sure that you have also downloaded and installed:
http://www.virtualbox.org/wiki/Downloads
VirtualBox 4.0.4 Oracle VM VirtualBox Extension Pack All platforms
Support for USB 2.0 devices, VirtualBox RDP and PXE boot for Intel
cards. See this chapter from the User Manual for an introduction to this
Extension Pack. The Extension Pack binaries are released under the
VirtualBox Personal Use and Evaluation License (PUEL). \

<http://download.virtualbox.org/virtualbox/4.0.4/Oracle_VM_VirtualBox_Extension_Pack-4.0.4-70112.vbox-extpack>

will open the VEP and offer to install in VirtualBox. Or in Oracle VM
VirtualBox Manager: File|Preferences|Extensions| right-click in the
window under 'Extension Packages' and select 'Add Package'. Navigate to
where you stored the downloaded extenstion package
(Oracle_VM_VirtualBox_Extension_Pack-4.0.4-70112.vbox-extpack) and
install from there.

When you have that installed: In the 'Settings|USB' window for your VM
Machine you should have these checked:

'Enable USB contoller'
'Enable USB 2.0 (EHCI) Controller'

You should have nothing in the USB Device Filters box. When you bring up
the VM you should have nothing ticked in the Devices|USB Devices menu.

Alternatively, you might want to try VMWare Player[1]. I use this on a
daily basis& typically only open/use VirtualBox for some older VM's
that I've had installed for quite some time.

I have it working now. Lucio's final bit of advice to try restarting the host system got it working for me in all my vm's.

Thanks, Jim
