Apache directory listings customization

I have recently been struggling with setting a custom header and footer
in Apache's directory listings.
after following the guide here http://howto.unixdev.net/dirlist.html and
restarting the service, the directory listings look exactly the same. any
ideas why this isn't working? my config file is available here
