Please try to post just the relevant bits.

What happens when you insmod or modprobe the module? If it appears to
succeed, does eth0 appear?

The only messages in your boot log which I noticed which could be
relevant are:

You may find it helps to try that, or to upgrade to the latest available
kernel.
