|of my smtp entry above ("host:smtp.earthlink.net" I'm just guessing here).

No, it's the other way around. Substitute the name of the host, i.e.,
smtp.earthlink.net where host appears, giving smtp.earthlink.net:smtp
The second smtp tells balsa to use port 25.
