Following instructions here, I checked CUPS and the Mozilla defaults. They
were letter. However, I uninstalled my printers and reinstalled them. I
made sure they were set to letter in CUPS and Mozilla and tried printing
again. They now print in the center of the page as they should, but the
headers and footers have disappeared. In the page setup menu they are set
up to appear and they do appear in Print Preview. They are also properly
set (at least I think) in prefs.js. It's a mystery.
