Following instructions here, I checked CUPS and the Mozilla defaults. They
were letter. However, I uninstalled my printers and reinstalled them. I
made sure they were set to letter in CUPS and Mozilla and tried printing
again. They now print in the center of the page as they should, but the
headers and footers have disappeared. In the page setup menu they are set
up to appear and they do appear in Print Preview. They are also properly
set (at least I think) in prefs.js. It's a mystery.

I managed to solve my similar problem in FC3 when I discovered that,
actually, there are two dialog boxes where the margins are set -- one in
File > Page Setup in Firefox and the second in Print > Properties in
Firefox.
In Print > Properties, I set all four margins to 0.5 inch. In File > Page
Setup, I set top and bottom margins to 1.0 inch, the left margin to 0.7
inch, and the right margin to 1.2 inch. The left-right margin settings
center the print area on the printed page. One would have hoped for
symmetry. Go figure.
Problem solved.
Now how do I change the font and font size in the header and footer?...
