FWIW and complete shot in the dark...
I had almost exactly the same problem with a Dell Optiplex several years
ago. Turned out to be a bad 2nd (backup) disk was apparently running out
of bad block allocations--but not enough to halt booting or low-load ops
(high-load gzip backups to this 2nd disk did it in regularly).
Go figure... :)
So I'm guessing hardware, if the software basically isn't in flux.
(Not that it helps you--just an example of how odd these things can be)
