YaST will set symlink to actual device, so try first is it working.
If YaST missed you can do that with
ln -S /dev/ttylt0 /dev/modem0
but I still don't see in YaST ltmodem package with driver for such modem.
Have you downloaded one?
Tried that one it acts like it is installing but fails at the end. Going03:0a.0 Communication controller: Conexant: Unknown device 2f20

You can have 14.4 kbps version for free. It is dog slow, but it is good
to test would it work with your modem. Later you can decide to buy
$20.-version, actually key that will enable full speed and few other
features.

The problem is probably "Conexant: Unknown device 2f20".
You have to see who is the vendor and the model. If there is no any marks on
modem, that it has at least FCC-ID number that can reveal the vendor and
model.
Where the problem can be?
On website http://www.linuxant.com/drivers/files/ on left hand side you can
see HCF and HSF modem classes and they wont work with wrong driver.
Did you meant that you'll try another type of driver, saying that you will
try new download.
