Trying it again, this time I clicked on the very top item on the patch
list (php5: security update). That added a click to several php5-... items
in the installed packages list that I did not have checked before, so now
it looks like I am going to install the whole batch of php5 items.
So I clicked accept and it went to the update download and install screen,
paused briefly while it went out to the update site, and then instantly
finished the installation. At least it used the linker when I told it to
close to inform various packages (about one third of the normal list).
So there is a problem with the update process not being able to tell if
things are updated. Or the the update process does not set all the flags
it needs to set. And even this time it failed to fix the problem: If I
pass my mouse over the round blue icon, it says that there is 1 update
available.

I remember problem, but I can't find any reference what was the cause.
Try in console as root:
zypper sl
and post the output. It will list configured repositories.
I had this problem once and could not make it go away. I
finally took the drastic step of deleting the package. Yast
told me that a load of other dependencies would be broken if
I did that but I forced it anyway.
Then I went to online update. Yast still wanted to download
the package so I let it. And that cured that.
But this is a drastic solution. If you chose it, be careful.
I agree that there may well be a bug here.
