Are you using the woody version of fvwm or a newer version?
Are you running X with 16 colors or less? The latest version of fvwm
seems to have a bug with that. If you start fvwm from failsafe mode and
get this error:
X Error of failed request: BadValue (integer parameter out of range
for operation)
Major opcode of failed request: 91 (X_QueryColors)
Value in failed request: 0xff
Serial number of failed request: 8
Current serial number in output stream: 8
try this:
LC_CTYPE=C fvwm -C pseudocolor &
or try another window manager.
