Above you say hda5 is /, so it can not be swap. If it is swap, than it can
not be /. One or the other, it can't be both.

In your BIOS you should have your hard drives set to LBA rather than CHS.

This shows two logical partitions, one should be swap. Which is it?

Do you have your / and swap partitions reversed?

You told us what you think your disk partitions are. There are
inconsistencies in what you say, and also the boot messages. We need to
know, for sure, what you really have. It would be a good idea to
insert a floppy into it's drive, post the results of;
# fdisk -l > /dev/fd0/part.txt <--that's a small L, and fd<zero>
A live CD, like Knoppix will have the fdisk app, as should the rescue
system on the first SUSE install disk. This will create a file, "part.txt"
on the floppy, that you can copy/past from, and add to your next post.
Also include the contents of /boot/grub/menu.lst. If you are posting from
a different machine, you can add this to the floppy by using the command;
# cat /dev/hda1/grub/menu.lst > /dev/fd0/menu.txt
