Where is that?
In the Linux kernel source tree.
you can also install the kernel documentation as a package. The
documentation will be in /usr/share/doc/linux-doc-<version>.
By the way you can use the command MAKEDEV (yes, it's in caps) to
recreate devices without knowing the major and minor numbers.
