SCSI CRC and parity errors

Hello All,
We have ADAPTEC 7892A SCSI HBA on the host machine connected to
QLOGIC RAID controller.We have developed a high level SCSI
driver,which is responsible for sending out vendor specific SCSI
commands to the RAID device.We are getting the following errors from
the adaptec driver.The system hangs after we see this error.

Can anyone please let us know the reason why we get these type of
errors.
We have changed the SCSI cables and terminator without any
success.Any pointers for resolving this problem would be appreciated.
