I notice that lspci shows an Intel ICH5 as the on-board IDE controller
but dmesg doesn't show that it has been detected. It would appear your
kernel doesn't support the ICH5.
If this is a recent (and custom-built) kernel, rebuild a new kernel
ensuring that you include support for your chipset (the Intel PIIX
driver should do) along with support for your other (currently working)
add-in IDE card. Be forewarned that this may change the order in which
hard drives are found. Your current hda could end up as hde or something!
OTOH, if this is an older kernel it is possible that it is simply too
old to recognize the relatively new ICH5 chipset. Upgrading to a more
recent kernel would be in order in that case.
