Where can I find some reference about the /proc file system for Debian?

I need to parse the /proc info for Hard disks and memory.
The memory part is really easy, but I'm having a hard time findind the
hard disk part.
I need something like this:
Hard disk Physical info like model and serial.
then in that hard disk what are the current partitions?
then in that partition what are the current mounts (Used Space, Total
Space and stuff like that)
