If you know that X is using values in the proper range for your monitor, yet your monitor complains about them being 'out of range', I would suspect a hardware problem. It could be as simple as a bad cable. It could an indicator that your monitor is not functioning properly. Or it could be that your graphics card is not putting out the sync signals properly.
Can you try a different monitor, or connect that monitor to a different computer? Trying a different cable is also worth a shot.
