Make/model? This one isn't in any of the HOWTOs, and the PCI-ID
code list (http://pciids.sf.net/) only says

There are three types of loosemodem driver. There is a 'binary only'
type, often from manufacturers or other who don't want to give away
their "trade secrets". These tend to be made for some specific Linux
distribution and release, and don't work on other distributions, or
other releases from the same distributor. They may not even work
if you update your kernel for a security fix. A second type is
free, or low cost, comes with (some) source, but lacks the later
protocol drivers (v.34, v.34+, v.90, v.92) and is thus limited to
speeds of 14.4k. The third type is a for-pay driver from someone
who includes the full capability (v.90 or even v.92).

Just make sure whatever you get is a v.92 modem. As that appears to be
the common and final protocol in use today. Flex / X2 / and even v.90
probably won't work well with most ISPs.

Depends on what you mean by "work well". v.90 is well supported (a bit
better than v.92), but the old Flex and X2 protocols were manufacturer
specific and were never adopted. Those modems will still work, but only
at speeds up to 33.6 (which is better than the free/low-cost winmodem
drivers). I still have several modems, and use them as backups. My
problem is that the wiring for the phones in the house is absolute crap
(I think it's paired wet string), and this limits the speeds even with
v.90 or v.92 modems to 26.8 to 33.6 max (worse in wet weather).
