It's not off when you are reading the temperature, it's been on long
enough for you to get into the BIOS which is probably 30 seconds or so.
If the heatsink is filthy or badly seated then the CPU can heat up really
fast.
Not if it's filthy. The heat capacity of the heat sink itself is high
enough that this does not happen so fast. However, if the heat sink
is badly seated or the thermal compound no longer conducts the heat
well, then only the thermal capacity of the CPU itself comes into
play, and then the CPU can heat up quickly. However, in that case I
would expect the CPU to heat up beyond the shutoff threshold in a
short period.
