Unless I seriously missed something since I started using
xterm in around 1987, handling those characters would be
handled by the shell and text-mode application programs
rather than the terminal emulator. There's a chance you
might be able to use a window manager or some other program
to detect the key events and perform the tasks you want, but
then you'd lose those keys for (just about) anything else
that uses them.

If the middle mouse button isn't working well, turn on "simulated middle
button" with the option "Emulate3Buttons" "on" in the inputdevices section
of your xorg.conf
What this does is trigger a middle button press when you press left and
right together.
Or, you could always just buy a new mouse.
