2) that is no longer much of an issue - most makers support USB mass
storage protocols - plug it in and it shows up as a removable drive.
But some of them don't default to that mode. And finding that option in
the menus can be tricky.

Also some have onboard memory that they use before using a card. Which
can only be accessed from the camera, and not a card reader. But most of
them do support the usb-storage access method. At least the newer ones.
Definitely use a card reader if you can. Using a cable really drains the
camera batteries. Which generally don't come with a power adapter.
