Reinserting does not help.
So far, all I can do when it happens is reboot.
It occurs both on PS2 and USB, with different mice.
So to me it seems the mouse handling in X is doing something quirky.
And no, restarting X does not help either.
What about "rmmod psmouse ; modprobe psmouse" (when you're using a PS/2
mouse)?
