That _looks_ like a Rockwell HCF winmodem. You can try the linmodem
web site (see the HOWTO below), but I really doubt it's worth the effort.
The same happens with an external modem www.chronos.com.tw Smartscm/56.
I can't see anything there - my browser complains that there is Big5,
rather than an ISO-8859 character set. Is this a USB modem perhaps?
A google search for the Smartscm/56 turns up multiple hits relating to
a Rockwell/Conexant SmartSCM/56 chipset which doesn't look promising.
Does your system have a copy of the Modem-HOWTO installed?
